Form 1095-C, Employer Provided Health Insurance Offer and Coverage provides coverage information for you, your spouse (if you file a joint return), and individuals you claim as dependents had qualifying health coverage (referred to as 'minimum essential coverage") for some or all months during the year.
Note. Employers are required to furnish only one Form 1095-C for all individuals whose coverage is reported on that form. As the recipient of this Form 1095-C, you should provide a copy to other individuals covered under the policy if they request it for their records.
Form 1095-C is not required to be filed with your tax return. If you had full-year coverage for 2019 no action needs to be taken with Form 1095-C. If you did not have full-year coverage, use the information on Form 1095-C to report the months of coverage you did have.
