Form 2441 - Dependent Care Benefits Not Provided

If your employer provided dependent care benefits under a qualified plan, but there is no amount in Box 10 of Form W-2 Wage and Tax Statement, you should contact your employer to determine if they need to issue you a Form W-2-C Corrected Wage and Tax Statement. The IRS will receive a copy of the Form W-2-C, if any, from the employer.

If you file Federal Form 2441, Child and Dependent Care Expenses, the IRS will match the amount reported on Form 2441, Line 12 to the Form W-2 (or Form W-2-C), Box 10 amount received from your employer.
