Include the following paid preparer information on all paper and electronically filed returns, if applicable:
  • Preparer's name
  • Firm's name
  • Address
  • Preparer's NYTPRIN or NYPTRIN exclusion code (if applicable)
  • Preparer's PTIN
  • Firm's EIN
  • Preparer's signature
